The assessment form collects your contact details, business name, sector, postcode, team and location sizes, preferred appointment, and any device counts or IT information you choose to provide. Please do not send patient or client records, passwords, payment information or other confidential material.
Why we use it
We use these details to respond, understand your enquiry, arrange an assessment and manage our discussions with you. Where applicable, this is to take steps at your request before providing a service, or for our legitimate interest in responding to business enquiries. The form does not sign you up for marketing.
How the request reaches us
Our website is hosted on Cloudflare. The assessment form sends your details to FormSubmit, which processes the submission and forwards it by email to contact@zystech.co.uk. These providers may also process technical information needed to deliver and protect their services. Keeping and sharing information
We use enquiry information for the purposes described above and retain it only for as long as needed to manage the enquiry, any resulting business relationship and applicable record-keeping obligations. Contact us if you would like to discuss retention or the handling of your details before submitting.
Your choices and rights
You can contact us to ask about access, correction or deletion of your personal information, or to raise an objection or request a restriction where applicable. If you have a concern, please contact us first. You can also raise it with the Information Commissioner’s Office.
Website features
The website does not set advertising or analytics cookies. The form service may use security checks when you submit a request. Appointment selection stays in your browser until you submit the form.
